Much like how, before the timeskip, gear 2nd caused Luffy extreme exhaustion and gear 3rd caused him to shrink, gear 4th puts massive strain on Luffy’s body. Unlike his fights against Doflamingo, Cracker, and Katakuri, there was really no reason for him to go gear 4th against Hody and Caesar.
