Do the Valar have physical forms?
There is every indication that they pick and design their forms themselves. All that is “predetermined” is their gender, which they will express and body forth when using any near-human shapes.

Why did the Elves leave Rivendell?
The Elves have to leave Middle Earth because they are diminishing and became weary because of the effects of Morgoth’s corruption into the world. They have to leave Middle Earth and go to Valinor which is the Arda not corrupted by Morgoth, and only the light of the Blessed Realm can sustain them.
Is Valinor same as Aman?
Aman is a continent. Valinor is the realm of the Valar, the inhabited part of Aman. There is also the island of Tol Eressëa off the eastern shore; taken together, the inhabited parts of Aman and Tol Eressëa are called the Undying Lands.
